Pristine Health Services Inc. will provide nursing services that focus on health safety and practices which reduce or prevent adverse outcomes for our individuals served. These services are identified in each individual’s plan which includes goals for the individual’s quality health outcomes. Nursing services include:

  • Nurse Consultation
  • Nurse Health Case Management
  • Nurse Case Management and Delegation

Pristine Health Services Inc.’s Community Living – Group Home Services was designed to provide individuals with development and maintenance of skills related to activities of daily living, instrumental activities of daily living, and socialization, through application of formal teaching methods in a community residential, Alternative Living Unit (ALU) or Group Home setting. Skills to be developed or maintained under Community Living – Group Home Services will be determined based on the participant’s individualized goals and outcomes as documented in his or her Person – Centered Plan.

Pristine Health Services Inc.’s Personal Support Services for individuals with developmental disabilities are guided by the Person – Centered Plan that is developed for each program participant, the program offers choices and increased independence while encouraging productivity and shared community involvement. Staff develop supportive and productive relationships with program Individuals while facilitating self-direction, self-advocacy, choice, community integration and natural support. Pristine Health Services Inc. will provide support delivered in the individual’s preferential manner to support that person’s independence in his or her own home and/or community in which he or she wishes to be involved in.

Pristine Health Services Inc.’s Transportation Services are designed specifically to enhance a participant’s ability to access community activities in response to needs identified through the participant’s person – centered plan. We provide non-medical assistance, training, and transport to a participant and the participant’s family caregiver.
